1. Forms - Athletic Clearance, Code of Conduct, Physical Form, Volunteer/Driver

2. Register for Auburn Ski Club Training and Purchase a Boreal Season Pass

3. Download and setup the GroupMe App

PVHS Athletics FORMS REQUIRED TO PARTICIPATE

Online Athletic Clearance, Code of Conduct & Physical Forms with insurance information are all required to participate on the race team.  A Parent and each athlete must complete.

Online Driver Clearance Forms - Volunteer/Driver  - turn any printed forms into the main office for final approval.

• Submit the "Volunteer Category C" Form

• Need copies of your Drivers License and Proof of Insurance

• Must be completed in order to drive students/athletes other than your own

Athlete Committed Code Night - Winter 2025-2026

All Athletes and a parent/guardian must attend the Athlete Committed Code Night for Winter Sports unless it was attended earlier for a Fall 2025 sport.  This must be attended each academic school year by all PVHS athletes and a parent/guardian.  There is a primary session on Dec 1, 2025 and a make-up date on Dec 8, 2025.  Athletes that do not attend Athlete Committed will not be allowed to participate in the upcoming season!  Please plan early to attend!
